we need to ensure that the copyright owner agrees to our publication of
the material. If you have a submission for us, please print and complete the
appropriate form(s) available at:

Click on Shtetlinks Donor Agreement form. Send 2 copies to Project Manager
(Chuck Weinstein) according to instructions.
